All registrants under 18 years of age are required to have a signed parental consent form on file.
- If the participant is under the age of 18, please provide the name and email of the parent/guardian when prompted. SNEMN is using DocuSign to collect parental consent forms. You will receive an email after you complete the registration process with instructions on how to complete the parental consent forms.
Medical and Model Release Form
- All participants MUST complete and sign the SNEMN Fine Arts Festival Medical and Model Release Form available Here: Medical and Model Release . The release form must be printed, signed and returned to Corrie within 5 days of registration.

Film, Video and Writing Entry:
- Writing entries All Writing, Film and Video Submissions must be submitted by February 7, 2024 using this form: https://form.jotform.com/232763378827166
